What is Access and Choice Wellbeing Support?
Access and Choice is a flexible and person-centered wellbeing support network. The service is provided by PHOs and NGOs that specialise in providing support for people with mild to moderate wellbeing needs. Priority population: Māori, Pasifika, and Youth
The support is intended as a wellbeing support and is provided through a partnership between General Practitioners, Practice Nurses, Health Improvement Practitioners (HIPs), Health Coaches (HCs), and Community Support Workers (CSWs).
